Chronic inflammation, also known as low-grade or systemic inflammation — can play a more puzzling and long-lasting role in the body. Consider the vast array of autoimmune disorders — such as rheumatoid arthritis, lupus and polymyalgia rheumatica — where the body's immune system mistakenly initiates an inflammatory response even though there's no apparent inflammation to fight off. Yesterday I was writing about how sunburn can inflame rheumatoid arthritis (RA), so today I thought I would provide more tips for living with rheumatoid Arthritis.
- Lighten your load - Every extra pound you carry puts four pounds of stress on your knees, and even a small amount of weight loss will provide relief. Talk to your doctor about the best program for you.
- Keep mobile - Careful regular and even vigorous exercise is critical for improving flexibility and maintaining muscle to support joints, but opt for low-impact exercises like swimming, biking and yoga that let you work out without painful pounding (and go especially easy during a flare). Avoid lifting heavy weights, which can be hard on tendons around the wrist. You might even consider a few sessions with a personal trainer who has experience with RA patients.
- Protect your heart - Because RA involves inflammation, it increases your risk of cardiovascular disease. Keep your heart as healthy as possible by maintaining normal blood pressure, avoiding smoking and keeping cholesterol levels under control.
- Build strong bones - Corticosteroid treatments can increase your odds of developing osteoporosis up to 50 percent. Prevent bone loss by getting 1,000 mg of calcium per day. Supplements are an option, but the best sources of this mineral are milk, yogurt, broccoli, kale and salmon.
- Deter depression - Living with a chronic pain and inflammation is challenging on many levels. Depression affects approximately 20 percent of people with RA and can actually worsen symptoms. Ask your doctor about treatment options or a referral to a mental-health professional.
- Get a flu shot - The flu is especially dangerous for anyone who has a chronic illness or an immune system suppressed by drugs. The Centers for Disease Control and Prevention (CDC) recommends that people who have autoimmune diseases get an annual flu shot.
- Soothe pain your way - Some people find a warm pack or bath soothing and pain relieving. Others prefer cold, which decreases blood flow to the inflamed area and eases muscle spasms. You can buy hot and cold packs from a drugstore, or use a hot water bottle or a pack of frozen vegetables.

One thing I have learn from living in the tropics is to take care to avoid sunburn, although I do occasionally get caught out.
Sunburns are the worst thing ever. But they are even more painful for those with a chronic inflammatory disease like rheumatoid arthritis. A sunburn is an acute inflammatory reaction of the skin that follows excessive exposure to ultraviolet radiation (UVR) from the sun. So basically, if your skin gets burnt it sends a trigger to you immune system (often being suppressed by a biologic medication) to hurry up and heal the burnt area.
But that's the last thing you want to happen — because it can send your body into a full flare-up mode. you could feel like a big puff ball, as your body responds to the inflammation started by sunburn.

Why do we get disease? Is it just a matter of time before each and every one of us develop some type of chronic disease? New research has shown for the first time that omega-3 in fish oil (DHA/EPA) could substantially and significantly reduce the signs and symptoms of osteoarthritis.
"There was strong evidence that omega-3 influences the biochemistry of the disease, and therefore not only helps prevent disease, but also slows its progression, potentially controlling established osteoarthritis,"
said lead researcher Dr John Tarlton.
The research was conducted in a strain of guinea pigs that naturally develop osteoarthritis and compared to controls. Fish oil intake reduced the pathology of tissue damage associated with osteoarthritis towards those without the problem. Medical research director of Arthritis Research UK, Professor Alan Silman, said:
"The possibility that omega-3 fatty acids could prevent osteoarthritis from developing has been a tantalising one. Some limited, previous research in dogs has suggested that we were a long way away from understanding the potential use in humans. However, this current research in guinea pigs is exciting as it brings us closer to understanding how omega-3 might fundamentally interfere with the osteoarthritis process, and that it could potentially be taken as a treatment."

Researchers at the David Geffen School of medicine at UCLA have discovered that negative and competitive social interactions increased pro-inflammatory proteins in the body.
The study of 122 healthy young adults who kept a journal of all positive, negative and competitive social interactions for eight days. the participants were then brought in for stressful 25 minute tests, where they did maths problems in their heads and gave speeches in front of strangers. Scientists took check swabs samples before the test , and 25 and 80 minutes after the test. they found that those with negative interactions during the week had high inflammatory protein levels both before and after the stressful mental test.
Interestingly, the negative interactions produced a different pro-inflammatory protein than competitive interactions.

Researchers from the University at Buffalo, State University of New York found that after a 900-calorie fast-food breakfast, study participants showed high blood levels of inflammation for the next four hours.
Emotional stress – Hormones produced as part of the stress response – such as cortisol and adrenaline – can lead to inflammation. Plus, cortisol can lead to belly fat, which produces inflammation of its own.

More than 90 percent of people with lupus will experience joint and/or muscle pain that can be disabling. Lupus causes inflammation in the synovial tissues inside joints of fingers, wrists, feet, ankles and knees. As a result of this inflammation, non-erosive arthritis can develop that causes pain, stiffness and swollen joints. Decades before my lupus was diagnosed, arthritis was one of the first symptoms of my lupus that appeared as early as my kindergarten years, and recurred all through high school, college and early adult life.
Medications, both over-the-counter and prescription drugs, are used to used to control the pain and inflammation associated with lupus. Medications for inflammation include non-narcotic analgesics/anti-inflammatory medications such as aspirin, ibuprofen, naprosyn, sulindac, and non-steroidal anti-inflammatory drugs (NSAIDs). Pain medications range from simple aspirin or acetaminophen to strong narcotics used for the most severe lupus pain. Persistent or severe inflammation at times requires the use of steroid medications to reduce swelling in involved joint tissues.
The Lupus Foundation of America provides the following information on NSAID medications used to treat painful inflammation.
Pain and inflammation are common in people with systemic lupus erythematosus (SLE). Sometimes these symptoms indicate serious organ involvement which may require powerful anti-inflammatory and immunosuppressive drugs, such as steroids (cortisone, prednisone).
At other times the inflammation is not as severe or does not affect major organs, and a less potent drug is indicated. In these cases, other milder anti-inflammatory and analgesic drugs can be used, especially a group of drugs called the non-steroidal anti-inflammatory drugs (NSAIDs).
While NSAIDs are not approved specifically for SLE by the Food and Drug Administration, they are approved for use in many musculoskeletal pain conditions such as arthritis and tendinitis, which also afflict people with lupus.
Other approaches to dealing with pain may include various therapies such as applying heat, gentle exercise to stimulate pain-relieving nature endorphins, prayer, meditation, bio-feedback and other non-chemical alternative approaches to pain management.

You tell yourself you are too young for this. RA can hit when you're 30 or any time later. It can affect men and women. But there are 2 1/2 times more women suffering from it. It's a long-term disease that leads to inflammation of the joints and surrounding tissues. It can also affect other organs.
Turns out, even a 5% weight gain could trigger it in someone who never had it. Inflammation. Now, they're saying with obesity on the rise –more & more people will get it. And they say the cause is unknown.
I'm thinking if even a slight weight gain triggers it, that's a clue. Some food or something in a certain food can be triggering it. Try the process of elimination to try to figure that out. One more thing…I say look at the 4 E's first whenever your health is off-balance.
- Emotions: Eckhart Tolle, the author of "The Power of Now" says anger affects your physical health when you repeatedly think about something that happened in the past or you worry about the future.
He says those thoughts cause negative emotions, which cause physical pain. It's the reason depression hurts or bullying. Negative words hurt. Literally. Negative thoughts hurt. Literally.
Tolle says anger is contagious. No one should be walking around angry.
Take the time to release it. You benefit. Everyone around you benefits.
Think of a record. If it has scratches, it skips. If it skips, you don't keep listening to it. If you did, it would severely damage the record (physical pain). The record is your mind. Change it.
Meditate on the present moment. Empty your mind of all thoughts. It's hard to do. Keep trying. Go to a quiet place. Close your eyes. Visualize releasing mental, emotional and physical pain. Focus on soft music, rain drops or simply your breathing.
- Environment – You may not be able to change your environment, which is why eliminating anger is so important. If you have to stay in an existing negative environment, go to a different room, step outside or go for a walk in nature. Turning off the computer and all electronic equipment helps too.
- Exercise
– The best exercise for RA is stretching slowly first thing in the morning, walking, yoga and swimming.

Sometimes, we can be overwhelmed by the various types of stress that come with everyday life. Today let's look at just one type of stress — emotional — to see how the brain and body respond. Suppose you get a letter from the government tax office saying you owe 40,000. One might be the release of insulin, a pro-inflammatory hormone.
Too much stress over an extended time period will cause insulin resistance, which is associated with obesity. That obesity (which itself creates inflammation) triggers the body to produce another pro-inflammatory hormone called cortisol, which creates more stress, and so on and so on. Clearly, then, controlling stress will help break this cycle of inflammation, and help the body return to its natural, balanced state.
Keys to stress management
The first step toward managing stress is to identify the things in your life that are causing it: marital problems, work pressures or illness in the family. Once you identify and understand these stressors, you can begin to find ways to change your environment and manage them. Support groups, exercise and certain nutritional supplements may help modify the levels of stress hormones and help the mind and body to re-balance.

Continuing on from yesterdays article, here seven warning signs of arthritis,and don't think for a minute that you arthritis is just for the old. Half of those who get it are under age 65. One in five adults — 50 million Americans — has been diagnosed with arthritis. Most wait to see a doctor until pain interferes with daily life — but pain isn't the only sign of trouble.
"Early is better with arthritis diagnosis," says Arthritis Foundation Vice President Patience White, a professor of medicine and pediatrics at the George Washington University School of Medicine and Health Sciences in Washington, D.C. Treating the autoimmune disease rheumatoid arthritis (RA) within the first months of onset, for example, can minimize joint deformities and even put the disease into remission, thanks to the latest treatments. With osteoarthritis (OA), a degenerative joint disease that's the most common kind of arthritis, the sooner you start behavioural changes, the better you may be able to manage pain and preserve mobility, White says.
Here's what to watch for.
- Do you find that you are unable to move around easily for the first half an hour on waking up?
Everybody has some morning stiffness, but normally it fades as you stretch and start moving. "With rheumatoid arthritis, it can take 30 minutes or more to loosen up — sometimes hours, or even all day," says Chaim Putterman, chief of rheumatology at the Albert Einstein College of Medicine and Montefiore Medical Centre in New York City. "People affected say they feel encased, like prisoners, and the feeing of being unable to move can be even more burdensome than the actual pain."
- Do your Your knee joint lock or send stabs of pain when you bend it, as when going up or down stairs. Added red flag: Knee pain if you're overweight."Every extra pound you gain feels like four pounds across your knees,"White says. Excess weight raises your risk of developing arthritis, which some 60 percent of obese people develop.
Related types of painful physical function include limping, being unable to extend your elbow, changes in how steadily you can stand or walk, and trouble standing on tiptoe.
- Chronic tiredness, loss of appetite, weight loss, anaemia, and/or fever that persists for weeks (longer than a bout of flu). Some combination of these symptoms usually appears, along with stiffness and pain. You might even notice changes in nonjoint tissue, such as eyes that feel dry and sore and may be red. These symptoms can come on gradually or suddenly.
Why pay attention: Having these mild, flu-like, across-the-body symptoms, along with stiffness and pain, points to rheumatoid arthritis. RA is a disorder of systemic inflammation, meaning the entire body is affected (as opposed to the problem being isolated in a particular joint)"You shouldn't just take two Tylenol and sleep the discomfort off," Putterman says. These symptoms warrant a physical exam soon.
- Do You have a joint that suddenly hurts like crazy — many sufferers give it a ten on a pain scale of one to ten, likening the pain to kidney stones, White says. The joint is likely also red, hot to the touch, and tender. Although this pattern can happen to other joints, the big toe is the most common site. Usually only one joint is affected at a time.
Why pay attention: Sudden, excruciating pain, especially in the large toe, is likely an attack of gout, the second-most common form of arthritis and the most common type of inflammatory arthritis. Gout occurs when uric acid, a natural waste product that circulates in the blood, builds up in body tissue as needle-shaped crystals. This happens either because the body produces too much uric acid or doesn't dispose of it well through the urine.
- Look for Bony spurs (small enlargements) on the joint of fingers. One may appear on the last joint, where it's known as a Heberden's node, or on the middle joint, where it's called a Bouchard's node. Sometimes the knobby bump appears at the base of the thumb (though this bump wasn't named after the doctor who studied it).
The joint is probably also stiff to move, although not necessarily painful. To the touch, the bump feels more like a bone than like a sponge. Some toes can also be affected.
Why pay attention: Doctors use this visual information to help diagnose arthritis, along with the rest of a physical exam, a history, and, sometimes, X-rays and lab tests. Heberden's nodes and Bouchard's nodes, seen in osteoarthritis, tend to run in families, especially through women, White says. Farmers and others who engage in a lot of manual labour are especially prone to OA of the lower extremities.
- Does pain stop you falling asleep, or do you wake up during the night with joint pain . Some sufferers begin to avoid family and other activities they enjoy because they feel ground down by chronic pain; the very prospect of visiting grandchildren or going to a social outing feels overwhelming.
Osteoarthritis pain is caused by eroding cartilage, which eventually causes bone to rub against bone — and yes, it can really hurt. Everyone has a different threshold for pain. But pain that interferes with your ability to enjoy the most basic, restful activities of life like sleep or enjoying your family? That's a different animal."It shows that what you've been doing before is not adequate and that the pain should be taken care of," Putterman says.
- Achy, hard-to-use hands
Trouble managing fine-motor skills: buttoning a shirt, tying shoelaces, using a fork and knife, turning a key in a lock, grasping a doorknob, snapping fingers.
Affected joints can be redder than surrounding skin, warm to the touch, and tender.
Why pay attention: “Trouble with these activities of daily living all suggest something worrisome is going on,” Putterman says. Many different joints in the hands and wrists tend to be involved with rheumatoid arthritis, making these hand and finger tasks so frustrating. Rheumatoid arthritis is also a symmetrical disease, meaning both sides of the body (and hands as well as feet) tend to be affected simultaneously. (In osteoarthritis, the joints affected usually aren't symmetrical.)
Notice which part of the hand is affected. Rheumatoid arthritis tends to afflict the knuckles and the next joint up, White says. In osteoarthritis, the top joints and the base of the thumb are more affected, and not necessarily all of the digits at once.

Arthritis literally means joint (arthro-) inflammation (-itis). Indeed, there is often inflammation around a diseased joint. But, did you know that arthritis is a disease of dehydration? Inflammation occurs when a special cell in your joint bursts and releases acids and histamines. This cell is called a mast cell. Researchers at California's Cedars-Sinai medical centre have unlocked the mystery of how an inflammatory molecule is produced in the body, a discovery they say could lead to advances in the treatment of rheumatoid arthritis, Type 2 diabetes and numerous other chronic diseases that affect tens of millions of people.
The study, funded by the National Institutes of Health, is published online by the peer-reviewed journal Immunity and appeared in the March print edition.
The researchers identified for the first time the mechanism that leads to the production of the molecule interleukin-1beta. It is a major contributor to inflammation, which lies at the root of many serious health conditions, including atherosclerotic heart disease and some types of strokes.
Current drug therapies seek to block this molecule's action after it is secreted by cells. However, the new research could lead to the development of treatments that would prevent the body from producing it, resulting in more effective medications and therapies for inflammatory diseases.
If we understand how this molecule is made in the body, we may be able to block it before it is produced,
Said study author Dr. Moshe Arditi, executive vice chair of research in the Department of Pediatrics and director of the Division of Pediatric Infectious Diseases and Immunology. Until now, this was the missing piece of the puzzle.
Arditi, who also directs Cedars-Sinai's Infectious and Immunologic Diseases Research Centre, found that damaged mitochondrial DNA activate specific proteins within dying cells, triggering the release of interleukin-1beta. Previous research has shown the molecule, when over-secreted by cells, can be a significant contributor to major inflammatory diseases.
Three of these diseases alone atherosclerosis, Type 2 diabetes and rheumatoid arthritis affect an estimated 100 million Americans.
Arditi is planning further studies to build on the findings.
The discovery by Arditi and colleagues has great potential to impact a wide range of inflammatory diseases, particularly in their early stages where an intervention could prevent more severe and debilitating ravages of such diseases,
Said Dr. Leon Fine, Cedars-Sinai's vice dean of research and chair of biomedical sciences. This discovery, at last, may open the door to such therapy.

Inflammation is the source of many diseases and some people, go from doctor to doctor, prescribed different medications. Myositis Pain and Inflammation
Inflammatory myositis is among a group of autoimmune diseases characterized by skeletal muscle weakness, elevated serum creatine kinase, and abnormalities seen on electromyography. These disorders often require long-term immunosuppression, usually with corticosteroids and second-line agents such as methotrexate.
However, prolonged use of steroids has been linked to numerous adverse cardiovascular side effects as well as to osteoporosis, cataracts, and increased risk of infections.
Epidemiologic studies have identified a strong association between atherosclerosis and other systemic autoimmune disorders such as rheumatoid arthritis and systemic lupus erythematosus, but the cardiovascular risk profile of patients with inflammatory myositis has not been previously studied.
The researchers initially set out to document the complications of corticosteroid and immunosuppressive treatment among patients with myositis. So Limaye and colleagues conducted a review of medical records and pathology reports from 344 patients who were enrolled in the South Australian myositis database between 1980 and 2009.
However, they found themselves
"surprised by an extremely high background prevalence of hypertension and diabetes mellitus in our cohort,"
the authors wrote.
The most commonly used immunosuppressive drugs were methotrexate, used in 83 patients, and azathioprine, given to 48 patients. Smaller numbers received hydroxychloroquine, cyclosporine, mycophenolate mofetil, and intravenous immunoglobulin.
The researchers first considered the presence or absence of comorbidities regardless of the timing of treatment in relation to a diagnosis of myositis.
In addition to the high prevalence of hypertension and diabetes, osteoporosis was also more prevalent in those with myositis compared with the overall Australian adult population (32% versus 3.4%).

You can do you best to prevent the pain from getting unmanageable by following a few simple practices.
- Prayer. Begin every day in prayer. The quiet, calming effect of meditative prayer is relaxing and encourages deep breathing.
- Warm bath or shower. Shower in the morning. The warm water helps relieve aches and pains and aids flexibility. After a day of strenuous physical activity take another one or soak in the tub.
- Moderate Exercise. Take a walk every day. This is beneficial on many levels. It gives you time to think away from the distractions of home and home office, you get exercise, when outside we absorb sunshine and fresh air, and can enjoy the scenery and people around us.

- Friends and Family. Time with loving, supportive friends and family encourages optimism, laughter, and feeling loved. It also encourages thinking of others rather than myself.
- Soft Music. Listen to classical or other instrumental music which can be inspirational and relaxing whilst working.
- Avoid Stress. Stress is believed to aggravate all illnesses but avoiding it is a tremendous challenge in this fast-paced world. Try to limit unnecessary commitments and over-scheduling.
- Sleep. You body needs sleep and you will feel better after a good night sleep.

Why is Inflammation a Problem?
